Diabetes Management Program
Our Diabetes Program provides education, tools and ongoing support for patients with Type 2 diabetes, and prediabetes. Our program centers on patient self-management, therefore we strive to help patients become an active participant in their health care.
What services are provided by the Diabetes Management Program?
Individual or group diabetes education and support for people with Type 2 Diabetes, provided by Certified Diabetes Educators (CDE) who are Registered Nurses (RN), Registered Dietitians (RN) and Registered Pharmacists (RPh) specializing in diabetes care.
Group Education
Group education consists of a 1.5 hour virtual class that engages you in a discussion on diabetes management.
How to prepare for Individual Appointments?
At your individual appointment with the Diabetes Team, the following points will be discussed:
- Your lifestyle (diet and physical activity). Please, fill out this Food Record if you have time
- Medications and medical history
- Health indicators, such as your weight and blood pressure
- Blood sugar levels and monitoring
Your team will work with you to set lifestyle goals that will aid your progress towards effective diabetes management. The ultimate goal of our diabetes management program is to prepare you to confidently control your diabetes.

Individuals living in Mississauga-Halton area can initiate a referral for diabetes education by calling Mississauga Halton Central Intake Program at 905 338 2983 ext. 4872 or visit their website.
