ABOUT US

The IPCT brings their expertise into the community to provide care where gaps in health care services may exist. Our IPCT offers services such as:

• acute/chronic disease management with our nurse practitioner
• mental health counselling with our social worker
• medication management with our pharmacist
• immunization clinics with our registered nurses at one of our community hubs
• Seniors Care
• Dementia Care
• Palliative Care
• Health Promotion Programs
