Family Health Teams consist of family physicians, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best primary care, when you need it, as close to home as possible. Family Health Teams improve access to health care across Ontario. These new teams will also help to keep people healthy and improve chronic disease prevention and management. Your enrollment in a Family Health Team is voluntary.
