Job Title: Pharmacist
Reports To: Executive Director
Job Type: Temporary part time 0.4 FTE
CVFHT is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging inter-professional primary care team dedicated to continuous learning and excellence within primary care with a demonstrated track record of impactful quality improvement activities. It is a not for profit organization that receives funding from the Ministry of Health and Long Term Care and provides primary health care services to over 12,000 patients in Mississauga. The CVFHT also has a mandate to provide French language primary care services to the Francophone community.
ROLE DEFINITION
To provide pharmaceutical care to patients by optimizing medication therapy and acting as a resource to the team. To ensure safe, comprehensive and effective medication usage through education on correct dosage and method of administration, adverse reactions and cross reactions with other medications. To participate in clinical activities under the direction of the FHT.
LIST THE SPECIFIC DUTIES OR RESPONSIBILITIES
- Prescribing Optimization:

Perform medication assessments/counseling for:
- patients with chronic diseases or chronic medications (on 3+ medications) Set up patient Med Profile (Goal- CVFHT EMR matches community EMR and also matches what patient is taking)
- Patients with multiple medications identifying drug related problems (DRP’s) such as incorrect dose, frequency, duration of therapy, lack of therapy, duplication or unnecessary therapy, interactions (drug/drug, drug/food) , adverse drug reactions (ADR”s), maintain appropriate therapeutic drug levels, assess and aid in adherence, assess affordability (cost)
- as requested by MD/Resident, patient or caregivers

Perform medication review for:
- Appropriate indication, dose, frequency, drug administration, drug interactions, allergies, formulary compliance, cost effectiveness and duplication of therapy.
- Recently hospitalized patients who have had medication changes (post hospital med rec)
- Reconcile community pharmacy Med Rec’s and liaise with regarding recommendations/changes/discrepancies (update EMR)
- Pre Physical Med Review (prior to Periodic Health Exam)
- Reconcile EMR- based on specialist reports (respirologist, Cardiologist, Endocrinologist, Nephrologist etc)
- Operational Efficiencies
- Create clinic strategies for monitoring and prescription renewals (screen all prescriptions for updated pharmacy, correctness, validity of renewal request, compliance, recent BP check, up to date bloodwork)
- Optimize use of prescription writer in EMR (list of Favorites etc) (List of community pharmacies)
- Expanded Scope of Practice- chronic medication renewals x 6 months max
- Check for recent bloodwork
- Provide drug information and drug therapy educational opportunities
- Present specific therapeutic topics to staff, physicians and residents (weekly Infoblast Pharmacy Newsletter)
- Assume responsibility for information on drugs, disease prevention and health promotion to ensure safe and effective provision of pharmaceutical care
- Provide reminders and alerts regarding drug-related issues problems and drug regulatory issues and initiates the process to deal with drug withdrawals, warnings or advisories.
- Expedite drug procurement by working with patients drug plan, ODB, Trillium, supplying LU codes, applying for EAP (ICR/Section8/Section16)
- Provide patient education through individual or group sessions to help in the understanding and management of particular health issues using particular pharmacy and therapeutic agents
- Participate in CDM and Wellness program development and implementation and evaluation including Diabetes, Asthma, COPD, Healthy You, Exercise as Medicine (Exercise Prescription-pedometers, Therabands),Walking to Wellness and Smoking Cessation (OMSC, STOP study CAMH), Bonefit Osteoporosis Training/Education
- Teach U of T or U Waterloo Pharm D students as clinical rotation
- Participate in ongoing research related to patient groups of the FHT
QUALIFICATIONS
Professional/Education/Experience Required:
Formal, Minimum Education Required
- Bachelor of Science in Pharmacy/Pharm D
- Pharmacy Examining Board of Canada Exams
- Previous Related or Special Experience Necessary:
- Residency in Hospital Pharmacy or previous hospital experience an asset
- Community pharmacy experience an asset
Registrations/Membership Requirements:
- Ontario College of Pharmacists (License)
- Canadian Society of Hospital Pharmacy (preferred)
- OPA (an asset)
- AFHTO (an asset)
Professional/Education/Experience Preferred:
- Ability to work effectively with all members of the health care team
- Ability to work in an interprofessional team environment
- Ability to prioritize, manage time effectively and be flexible in a very active work environment
