Registered Dietitian - Bilingual

Job Title:   Registered Dietitian – bilingual 

  

Reports To: Executive Director 

 

Primary Function or Purpose of Job or Position 

 

CVFHT is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging inter-professional primary care team dedicated to continuous learning and excellence within primary care with a demonstrated track record of impactful quality improvement activities. It is a not-for-profit organization that receives funding from the Ministry of Health and Long-Term Care and provides primary health care services to over 14,000 patients in Mississauga. The CVFHT also has a mandate to provide French language primary care services to the Francophone community. 

 

The Registered Dietitian is a member of the clinical team of the CVFHT and works in collaboration with a team of physicians, residents, nurses, nurse practitioners and other healthcare professionals to co-ordinate the highest possible quality of care for patients when they need it.  

  

 

 Specific Duties and Responsibilities 

  

  • Triage and maintain internal and external referrals of patients daily
  • Provide self-management education and counselling support to referred patients and families 
  • Work collaboratively with other healthcare professionals to provide dietary support and education 
  • Assessment of patients to determine physical status, previous management and control, compliance with treatment and learning need
  • Provide individual counselling, teaching sessions and coaching, in addition to group classes, to meet the individual nutritional, behavioral and self-care needs of the patient and his/her family
  • Encourage participation and involvement of family members in the management of the chronic diseases and other complicating factors
  • Promote self care and peer support 
  • Maintain the appropriate documentation and communication electronically and ensure effective team approach to care and effective data capture with a focus on continuous improvement
  • Develop and utilize written and audio-visual teaching aids to facilitate adult learning for patients and families and to support the inter-professional team, particularly students and medical residents
  • Establish and maintain ongoing liaison with agencies and community resources that are relevant and necessary to the integrations of services and high-quality patient care
  • Provide teaching related to diet and nutrition to inter-professional team members, to help them in their practice
  • Contribute to, and participate in, overall Family Health Team planning, programming and evolution including participation in quality improvement activities. Participate in ongoing quality improvement and research related to client groups of the FHT
  • Engage in quality improvement activities to improve access efficiencies and customer service, as well as support of data collection
  • Other duties as assigned by the FHO lead physician  

 

Job Requirements: 

  • Registered Dietitian in good standing with the College of Dietitians of Ontario; 
  • Minimum of two (2) years of relevant work experience; 
  • Certified Diabetes Educator is an asset but not required;  
  • Strong knowledge about Family Health Organization (FHO) structure and demonstrated application of adult education principles; 
  • Previous experience in chronic disease education – both in individual and group settings; 
  • Experience working effectively in a busy clinical environment - primary or community settings preferred; 
  • Advanced MS Office skills - Outlook, Word, Excel and Power Point – and EMR syste, (Accuro) knowledge is required;  
  • Strong problem solving and excellent communication skills both oral and written; 
  • Bilingual in French is a requirement.

  • Demonstrated ability and commitment to establish positive and supportive relationship with patients, families and other health professionals.  

  • Satisfactory attendance and employment record.
